Smarter Diagnosis Management with Patient Problems

TriMed’s Patient Problems feature transforms how practices track, organize, and manage patient diagnoses. Designed for efficiency, this enhancement automatically groups similar diagnoses, keeps chronic and recurring issues visible, and removes the need for manual list maintenance.

With this release, your Diagnosis History chart header and Patient Problem List now work together seamlessly — letting you view related ICDs grouped under one patient problem name.

Smarter Grouping

The Diagnosis History header now automatically groups related ICD codes under shared Patient Problems (e.g., all H66 Otitis Media codes display together as “Otitis Media”). This simplifies the chart view and makes recurring issues easy to identify.

Automated Rules

Create rules to auto-add diagnoses to the Patient Problem List after they occur multiple times, and auto-expire resolved issues after a chosen timeframe — keeping the list clean and relevant.

In this example, Otitis Media is automatically added to the patient’s Problem List after the diagnosis appears three times within one year, and it will be marked as Chronic Resolved if no new diagnoses occur within two years.

Consistent Insights

TriMed now checks Patient Problem rules each time a diagnosis is added and updates the Problem List automatically when notes are signed. Persistent or recurring issues are surfaced so clinicians stay informed.

TriMed Transcribe

TriMed Transcribe enables users to convert audio recordings into text within the EHR seamlessly. The feature includes a dedicated sensor and a pop-up interface for quick transcription, with options to refine the text using the Polish function and copy it across multiple chart fields. All audio histories are stored, allowing users to transcribe recordings at any time.

Document Textract

The OCR Document Textract feature uses optical character recognition to extract key patient details, provider information, and summary content from uploaded PDFs. When enabled, it automatically populates the related patient and provider fields and presents a summarized view of the document. The feature also integrates with the Plan Predict API to suggest likely diagnoses based on the extracted summary, supporting clinical decision-making and reducing documentation burden.

New Immunization Interval Rule for Live Vaccines (FluMist, MMR, and Varicella)

TriMed has added a new system rule to ensure proper spacing for live-virus vaccines, including MMR, Varicella, and live influenza (FluMist®). These vaccines may be administered on the same day; however, if given on different days, they must be separated by at least 28 days. If administered before the 28-day interval, the later dose is considered invalid. Please note: the four-day grace period does not apply to the interval between two live-virus vaccines.

Setup Required: To enable this functionality, go to Admin > Immunization > Influenza > FluMist CVX Codes and add the appropriate FluMist CVX codes.

How it Works: If a patient has received FluMist, and an MMR and/or Varicella vaccine is ordered within 28 days, the system will display a note alerting the user, as shown in the example below:

Delete SubNotes

A new 'Delete SubNotes' option is now available under the More icon in a patient note. This feature allows users to remove SubNotes that were added accidentally.

Selecting 'Delete SubNotes' triggers a popup that lists all SubNotes that were added manually via right-click, Note top menu (Add Form or Add Merge), or the 'Quick Add' option.

Users can select one or more SubNotes to delete. Once confirmed, the selected SubNotes and their associated data are permanently removed from the patient note. This action cannot be undone.

The space from a removed SubNote will remain blank and is not automatically adjusted.

All deletions are recorded in the Audit Log, including the Patient Note Control IDs of the deleted SubNotes.

Patient Registration: Baby On The Way

Within RCM Admin > Enterprise > Company, when the 'Show Unknown as option for Sex' option is enabled, a new setting called 'New Patient – Allow to Add Baby on the Way' is now available.

When this setting is enabled in Company settings, a checkbox appears during patient registration. Selecting this option will automatically pull the guarantor’s last name, assign the first name as 'Baby1,' default the gender to 'Unknown,' and leave the date of birth blank. Instead of a date of birth, users will enter a due date.

When the baby is born, the EHR user can update the patient’s information by entering the correct name, date of birth, and gender.

Include Future Dated Chart Items on Chart Items Widget

The Chart Items widget now includes the option to display future-dated items. When the Date Type is set to Item Date, users can specify how many days into the future to show chart items by entering a positive number in the 'Future Items Date Range in Days' field. This setting helps users proactively monitor upcoming documentation or tasks.
